Step 1: Call for an Appointment

Step 2: Obtain Your Current Medical Records
Contact your physician and obtain a copy of your current medical records. These documents must include, at a minimum, your diagnosis (that qualifies you for receiving medical marijuana) along with records of your current treatments and medications.
These documents may come from your primary care physician or specialist. An example would be a copy of your office visit with your Neurologist that documents your diagnosis of Multiple Sclerosis or from your Gastroenterologist documenting your diagnosis of Crohn’s disease and your current treatment. A discharge summary from your physician may suffice if it includes your diagnosis and medications.
For patients with PTSD, an office note or letter from your Psychiatrist, Psychologist or Licensed Therapist would serve.
These records must be brought with you at your first appointment.
IMPORTANT NOTE: We MUST receive ALL of your required medical records and PA ID either prior to or at the time of your evaluation or you will NOT be seen.
Step 3: Arrive Early for Your Appointment
Arrive at least 15 minutes early to fill out required paper work. Download the forms above to speed up your registration process.
Bring Proof of Pennsylvania Residency. This may be a current PA Driver’s License, US Issued passport with a PA address or an official PA issued ID card (your ID must be current).
Bring your medical records.
Dress comfortably and appropriately for an examination.
Bring your payment method. We accept cash or credit/debit cards. We do NOT accept checks or insurance.
Allow adequate time for your visit.
Patients under the age of 18 must be accompanied by their parent or legal guardian.
Step 4: Create A Patient Profile
Visit the Commonwealth Of Pennsylvania website for medical marijuana patients and follow the instructions to create a patient profile in the patient and caregiver registry.
